Transcript Information


The Registrar's Office is closed during all school breaks. Transcript requests are processed only during regularly scheduled school days.

Request your transcript online through Parchment.com or by mail following the directions below:
 
ONLINE:
Place your transcript requests online with Parchment - the safe, paperless way to send transcripts directly to the destinations you choose. 

Track your request online and receive an email notification when we approve your transcript request, and again when your transcript is delivered electronically (or mailed to certain destinations).  Provided as a service to Indiana students and families by ISM Education Loans. This is FREE to most destinations nationwide for current students.  There is a fee for electronic or mailed delivery for graduated students.

BY MAIL:
Please print and complete the Transcript Request Form (PDF below).  There is a $2.00 fee for each transcript requested for a current student and a $5.00 fee for all other transcript requests (cash, check, or money order only, payable to Portage High School).  We require 2 school days to process all requests.  Mail the form along with the fee to:

GED TRANSCRIPT REQUESTS:
All GED transcripts must be requested from www.diplomasender.com
 
EDUCATION VERIFICATION REQUESTS: 
For quickest response, requests can be made through Parchment.com.
Faxed request can be made to 219-764-6063, Attn: Registrar.  A signed and dated release is required.
