The Centers for Disease Control and Prevention’s data show significant drops in routine vaccination coverage during the pandemic. Portage Township Schools is partnering with Porter County Health Department to host a vaccination clinic so students who need their required vaccines have the opportunity to receive them. Middle school and high school students may be seen at Portage High School on October 14, 2022 during the school day for school-required vaccinations with parental consent.

Please fill out the consent form, linked below, for your child and email to [email protected] or fax to 219-465-3531. Paper copies of the consent forms are also available in the nurse's office at your child's school. Transportation to and from PHS will be provided for middle school students who return the required consent forms; passes will be sent to high school students who return the required consent forms. If you would like your child to also receive a Covid vaccine or booster, please fill out the Covid vaccine form as well.

The deadline to submit forms is October 1.
